  • Applicant MUST sign the form in the space provided above Section 1 and in Section 10.
  • 申请人须在第1部分和第10部分上方空白处签名
  • Applications MUST be handwritten, using a BLUE or BLACK ink and in BLOCK CAPITAL letters.
  • 申请表需使用黑色或蓝色水笔手写填写，英文字母要大写。
  • Applicant MUST complete Sections 1, 3, 4, 5, 7, & 10 and if applicable Sections 2, 6, 8, and 9.
  • 申请人要求填写第1、3、4、5、7和第10栏，若个人情况适用于第2、6、8和9栏，也应填写
  • Applicants who are married, divorced or separated MUST also complete Section 2.
  • 申请人结婚、离异或分居，须填写第2栏。

Section 11:

  • The Recommender, MUST be a notary public or attorney at law and MUST also endorse the reverse side of ONE of applicant’s photos as indicated in the instructions on the first page of the application. See Section on Photographs of Passport Application  for  details  (“I  certify  that  this  is  a  true  likeness  of  the  passport holder Mr./Mrs./Ms ”).
  • 推荐人须是公证员或律师。推荐人须在申请人的一张照片背面书写证词，申请表首页填表说明中有此要求。请详细参阅护照申请关于照片的要求（“我证明此照片是护照持有人某某先生、女士的真实照片”）
  • The Recommender cannot be a relative (direct or in–law) of the applicant.
  • 推荐人不能是申请人的直系亲属或旁系亲属.
  • Two passport size pictures no more than six (6) months old and MUST be submitted with application.
  • 随申请表递交的两张申请人照片须是6个月以内的近照
  • The Recommender in Section 11 MUST endorse the reverse side of one of the pictures. See Item 6 above and PART A of the application form for details.
  • 第11栏的推荐人需在申请人的一张照片背面书写证词。具体请参阅上述6项和申请表的第一部分。

 Specifications for photographs:

  1. No glasses – 不佩戴眼镜
  2. No hair bands visible. – 发卡不外露
  3. No big earrings. – 耳环不能过大
  4. Ears must be visible.- 耳朵无遮挡
  5. Hair must be away from face and not covering the forehead. – 头发不遮挡面部和前额
  6. Chest must be covered. – 胸部不外露
  7. Children under the age of 16 years MUST have written consent of parent/legal guardian when applying for a passport. – 未满16岁的儿童申请护照，需提交父母或法定监护人的同意函。
  8. Applicants if married, MUST supply their ORIGINAL Marriage Certificate. If divorced please supply ORIGINAL Divorce Decree. 未满16岁的儿童申请护照，需提交父母或法定监护人的同意函。

Applicant MUST – 申请人须：:

  • Produce ORIGINAL Birth Certificate and – 出具出生证明原件和
  • Certificate of Naturalization or Registration (if applicable), and -入籍证书原件或入籍注册纸原件（若适用）以及
  • Must surrender current (whether expired or not) passport with application. 现有护照（过期或未过期）须随申请材料一起递交
  • Complete the passport application in its entirety. 完整填写护照申请表
  • Provide proof of payment from Bank- 提供银行出具的费用支付证明
  • NB/ 注意：: If applicant’s passport is lost or stolen, the applicant must go to the police station to get a note of evidence.– 若护照丢失或被盗，须报警并留存报警记录证明。

All documents will be returned with new passport. – 所有文件材料将和新护照一同返还申请人
